If you have never submitted your SSN/ITIN before, do the following:
- Log in to myHU (See MyHU Login : Harrisburg University IT Helpdesk if you are having issues with logging in)
- Select the Finance tab
- Click on Student Forms
- Choose the "SSN/ITIN Form"
Please note that you will not see your SSN/ITIN listed on myHU for security reasons.
If you need to resubmit your SSN/ITIN due to an error during the first submission OR you are switching from ITIN to SSN, please fill out Form W-9S. You may return your W-9S via the following avenues:
- Fax it to 717-901-3135
- Mail to ATTN: Business Office, 326 Market St, Harrisburg, PA 17101
- Return in person to the Business Office (14th floor).
For security reasons, please DO NOT email your W-9S or submit it to our support portal. Use one of the three options above.
